Peppermint: Traditional Use, Constituents and Evidence

My nan grew peppermint along the south-facing fence of her place in Stirling, tucked between lemon balm and a sprawling rosemary. She never called it a medicine, exactly. Just "something for after dinner." That instinct , the sense that this particular mint was different , stretches back a lot further than one Adelaide Hills garden.

A brief history of a very old plant

Peppermint (Mentha × piperita L.) is a sterile hybrid of watermint (Mentha aquatica) and spearmint (Mentha spicata), most likely first cultivated in England in the late 17th century, though related mints appear in Egyptian, Greek and Roman sources well before that. The Roman naturalist Pliny described mint as so stimulating to the mind that scholars wore crowns of it while studying , which I find charming, if probably optimistic.

By the 18th century, peppermint had entered the London Pharmacopoeia (1721 edition), and European apothecaries were distilling the oil commercially. In Australia, peppermint has a layered history: Mentha australis and several Eucalyptus species share the common name "peppermint" among First Nations communities and early settlers alike, so any reading of historical use here needs careful disambiguation. The introduced Mentha × piperita arrived with the first colonists and was quickly established in kitchen gardens across the colonies.

Key constituents: what's actually in the plant

The primary constituent most people know is menthol; a cyclic monoterpene alcohol that gives peppermint oil its characteristic cooling sensation. In a well-grown crop, menthol can comprise 35–55% of the essential oil by volume, though this varies considerably with cultivar, harvest time and drying method. Menthone, menthyl acetate, menthofuran, isomenthol and cineole (also called eucalyptol) make up most of the rest.

Beyond the essential oil fraction, peppermint leaf contains phenolic acids (notably rosmarinic acid), flavonoids (luteolin, eriocitrin, hesperidin), and modest amounts of tannins. The whole-plant profile is substantially different from isolated peppermint oil, a distinction that matters for understanding the research, which often uses enteric-coated oil capsules rather than leaf tea.

Menthol is also a terpene, the same broad chemical class that includes the aromatic compounds studied extensively in cannabis research: compounds like linalool, beta-caryophyllene and limonene. That shared chemistry is genuinely interesting from a phytomedicine perspective. It doesn't mean the plants work the same way; it means they share a biosynthetic family tree.

How menthol produces its cooling effect

The mechanism here is well-characterised. Menthol activates TRPM8, a transient receptor potential ion channel expressed on cold-sensitive sensory neurones. It essentially tricks the channel into firing a "cold" signal without any actual temperature drop. This is a receptor-level, pharmacologically understood action, and it's why peppermint oil applied topically produces a genuine sensory response rather than a placebo one.

TRPA1 channels (involved in irritant and nociceptive signalling) are also modulated by menthol at higher concentrations, the direction of that interaction (activation versus inhibition) depends on concentration, which partly explains why peppermint can feel refreshing at low doses and irritating at high ones.

Separately, cineole/eucalyptol contributes its own sensory and pharmacological character to the oil's overall profile, with documented activity on calcium channel signalling in smooth muscle; relevant context for understanding research into enteric applications, though not a basis for making clinical claims here.

The state of the research, and its real limits

The most studied application of peppermint oil in the clinical literature involves enteric-coated capsules. The Cochrane Database, which is about as good a reference as you'll find for systematic evidence synthesis, published a review (Pittler & Ernst, updated across editions) noting that several randomised controlled trials had examined peppermint oil capsules and gastrointestinal outcomes, with some positive signals in the data. But the trials are consistently small, heterogeneous in design, and variable in oil standardisation. The effect sizes are difficult to generalise.

Topical menthol has a larger and more consistent body of evidence, particularly in sensory research. The TRPM8 mechanism is not disputed. What remains less settled is how reliably a topical product delivers a reproducible dose to deeper tissues, and whether sensory channel activation translates to the functional outcomes people often assume it does.

Honestly, this is where I think some of the popular writing about peppermint goes too far. The mechanism is real and measurable. Jumping from "menthol activates a cold receptor" to "peppermint oil relieves tension headaches" requires clinical evidence at each step, and the headache trials, whilst interesting, are not large enough to be definitive. I'd want to see bigger, pre-registered replication before I'd lean on that claim.

For the leaf infusion (tea); what most people actually consume, the clinical evidence base is thinner still. The bioavailability of phenolic constituents from an infusion is a separate question from the oil studies, and the two shouldn't be conflated. Australian regulatory context

In Australia, peppermint products fall under the TGA's regulatory framework for complementary medicines. Most commercially available peppermint products, teas, tinctures, topical creams, and standard-dose essential oil capsules; are listed medicines on the Australian Register of Therapeutic Goods (ARTG) rather than registered medicines.

This distinction is significant. Listed medicines are assessed for safety and quality by the TGA but are not individually evaluated for efficacy before being placed on the market. Sponsors self-certify that their evidence supports the permitted indications, which are drawn from a pre-approved list. So when you see a listed peppermint capsule with an on-label claim like "relieves digestive discomfort," that claim has been permitted under the TGA's framework for complementary medicine indications, it does not mean the TGA has independently reviewed and confirmed that the specific product works.

Higher-dose or pharmaceutical-grade peppermint oil products, if presented as registered medicines with specific efficacy claims, face a different evidentiary bar. The standardised extract question matters here too: because menthol content varies with growing conditions, a product that specifies standardised menthol concentration is making a different quality claim than one that simply lists "peppermint oil" as an ingredient.

Good manufacturing practice (GMP) requirements apply to all TGA-listed and registered complementary medicines, which is at least a meaningful floor for quality assurance, even when individual efficacy hasn't been separately verified.

There are no specific import restrictions on peppermint equivalent to those governing, say, kava (which faces strict personal importation controls under Australian Border Force rules and TGA scheduling). Peppermint is not a scheduled substance under the current Poisons Standard. That said, concentrated essential oils, any of them; carry real safety considerations around ingestion at high doses, particularly for children and infants, and that is not a hypothetical caveat.

Traditional use and what it can and can't tell us

The traditional use record for peppermint is genuinely long and geographically broad. Ayurvedic, Unani, and European herbal traditions all document mint-family plants for digestive, respiratory and topical applications. Under the TGA's traditional use pathway for listed medicines, a documented history of use over a "significant period" (generally 30 years or more, including 15 years in a comparable Western country) can support a traditional use indication without the same clinical trial evidence required for an efficacy claim.

This is a reasonable regulatory framework, in my view, traditional use evidence is real evidence, even if it sits at a different level of the evidence hierarchy than a well-powered RCT. The mistake is treating traditional use and clinical trial evidence as equivalent, or treating one as a substitute for the other. They answer slightly different questions.

I spent a while last autumn going back through some 19th-century British herbals, Culpeper's is the obvious one, but the later Victorian manuals are more interesting to me because they show the moment when botanical medicine started trying to speak the language of chemistry. Peppermint comes up constantly. What you notice is how carefully the good authors separate "what people have observed" from "what we can explain." That's still the right instinct.
